Job Summary
We are seeking a dedicated Nurse Coordinator to join our healthcare team. In this vital role, you will be responsible for overseeing patient care coordination, ensuring that patients receive comprehensive and effective treatment throughout their healthcare journey. Responsibilities
· Coordinate patient care activities across various departments, ensuring seamless transitions and continuity of care.
· Conduct thorough patient assessments to develop individualized care plans based on medical history and current health status.
· Collaborate with interdisciplinary teams to facilitate effective communication and optimal treatment strategies.
· Monitor patient progress, addressing any concerns or changes in condition promptly.
· Educate patients and families on treatment plans, medications, and follow-up care requirements.
· Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards.
· Assist in the management of chronic conditions through health coaching and patient education initiatives.
· Participate in quality improvement initiatives to enhance patient safety and service delivery.
· Stay updated on best practices in nursing care, including advancements in technology and treatment protocols.
Skills
· Proficient in electronic medical record (EMR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
· Strong clinical skills including IV insertion, medication administration, triage, and acute pain management.
· Experience with specialized treatments such as botulinum toxin administration, dialysis, and suturing.
· Knowledge of ICD-9 and ICD-10 coding as well as familiarity with NCQA standards.
· Background in critical care settings including ICU or emergency medicine experience is preferred.
· Excellent interpersonal skills with the ability to work effectively with diverse populations, including those with developmental disabilities.
· Strong organizational skills with attention to detail for accurate medical documentation and patient assessments.
