RCM Health Care Services is partnering with a leading H ospice & Palliative Care program , a highly respected nonprofit organization, to recruit Hospice Social Workers (LCSWs) to join their growing Palliative Care & Hospice teams across NYC.
These roles are ideal for experienced Social Workers passionate about supporting patients and families navigating serious illness, end-of-life care, and complex psychosocial needs.
About the RoleAs a key member of the Interdisciplinary Team (IDT) , the Hospice Social Worker provides emotional support, counseling, case management, and community resource coordination to patients and families in a field-based capacity. You will conduct biopsychosocial assessments, develop individualized plans of care, and help patients access critical services that support quality of life.

Territory OptionsWe are hiring Social Workers , each with a defined region:
Manhattan (field-based, travel required)
Bronx (car & valid drivers license required)
Queens + Nassau County (car & valid drivers license required)
Serve as the social work representative on the Hospice & Palliative Care IDT.
Complete biopsychosocial/spiritual assessments within required timeframes.
Provide supportive counseling, behavioral interventions, and crisis support.
Manage mental health concerns, reduce emotional distress, and help resolve family conflict.
Conduct 2025 patient visits per week across assigned settings.
Develop social work plans of care with clear goals, interventions, and community referrals.
Support advance care planning (HCP, MOLST, Living Will).
Assist with Medicaid applications, M11Q forms, nursing home placement, DOH screens, and referrals.
Document timely and accurately in the EMR.
Participate in IDT meetings, discharge planning, and care transitions.
Provide education, advocacy, and culturally competent support to diverse patient populations.
Remain current with hospice regulations, CMS requirements, and evidence-based practices.
Supervise interns or provide peer supervision as needed.
Masters Degree in Social Work from an accredited program.
Active NYS LCSW and current NYS Registration.
Minimum 3 years of clinical experience in a healthcare setting.
Hospice, Palliative Care, community health, long-term care, or behavioral health experience is preferred.
Valid drivers license and vehicle ( required for Bronx and Queens/Nassau roles).
Strong communication, crisis intervention, and case management skills.
Ability to manage complex cases, navigate community resources, and support families during the dying process.
