Position Details
Title: Case Manager, Licensed Social Worker (LSW)
Location: Must reside in Clinton, Clermont, or Hamilton County, OH
Schedule: Monday-Friday, 8:00 AM - 5:00 PM EST
Work Type: Hybridmunity-based care role
Travel: 50-75% local travel to member homes or nursing facilities (mileage reimbursed)
Pay Range: $35.95 - $37.99/hour (based on experience)
Responsibilities
Conductprehensive assessments to determine member needs and eligibility
Develop individualized care plans to enhance wellness and independence
Facilitate member access tomunity and healthcare resources
Use clinical judgment to navigateplex health and social issues
Collaborate with multidisciplinary teams to ensure optimal oues
Maintain documentation in line with regulatory andpany standards
Qualifications
Active LSW license in good standing (State of Ohio)
3+ years of behavioral health or social work experience
Strongmunication,anization, and problem-solving skills
Reliable transportation and willingness to travel locally
Preferred: Case management experience,munity-based social work, and proficiency in Microsoft Office
