Programs are offered for children, adolescents and adults struggling with a wide range of behavioral health issues.

Job Summary

• Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence-based substance use and addiction treatment services. Maintains ethical principles and professional standards of practice in adherence to the disciplines professional code of ethics.

• Collaborates with the interdisciplinary team to conduct a comprehensive bio-psychosocial assessment, develops and implements treatment/discharge plan, monitors progress of interventions and revises plan to meet patient and family needs, and coordinates consults/referrals to remove barriers and promote the individualized care plan, detox, and/or substance use rehab.

• Provides education to patient/family with respect to meeting established goals, including but not limited to disease prevention and progression, advocacy for benefits, and maintenance of treatment regimen.

• Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, referral and discharge planning.

• Maintains timely, patient-centered documentation that reflects the patient and client systems pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.

• Participates in administrative and developmental activities including team meetings and clinical supervision by program manager.

• Works as a team member to contribute to improved patient experience.

• Master's Degree in Social Work, Counseling, Marriage and Family Therapy or related Human Service field
• Previous experience in a healthcare setting working with substance use disorders or addiction services
• LCSW, LADC, or LPC; responsible for maintaining current licensure
• Valid Driver's License
• Computer proficient
• Ability to assess problems, determine solutions, and make decisions within a timely manner
• Utilize supervision and team approaches in the context of patient assessment and clinical interventions
• Excellent communication skills, with the ability to establish relationships within and outside their entity
• Focused on personal, patient/client and unit/staff safety
