Purpose of Position:
Under the supervision of the Office Manager and in conjunction with the Clinical Manager, the Patient Service Coordinator is responsible for all scheduling tasks related to patients, and field staff (all disciplines) scheduled to work. The patient Services Coordinator is responsible for maintaining up to date schedules for 7 day a week operation. The patient Services Coordinator schedules requested Starts of Care, Resumption of Care ,Recertification, Revisits, Evaluations, and other visits for all clinical disciplines (SN, LPN, PT, OT, ST, MSW, HHA ) in a timely manner to ensure the highest level of patient care.

Responsibilities:
- Supports clinical manager in coordinating operations
- Coordinates team clinical activities to include, but not limited to:
- Keeps manager informed of team member availability and daily attendance
- Schedules all ordered visits with the appropriate visit codes, associated buddy codes to the correct personnel.
- Analyzes Managed care authorizations (if applicable) in terms of scheduling visits
- Ensures team members are aware of their schedules
- Maintain am and pm scheduling communication
- Initiates weekend schedule by obtaining from staff the names of patients to be seen on the weekend, performs confirmation phone calls of scheduled staff and anticipated patient visits. Assists manager in scheduling as directed.
- Completes the assigned HCHB workflows including but not limited to: Assign evaluation visit, complete requested episode for the first week, process missed, reassigned, rescheduled visits, Discharge and other OASIS data collection visits
- Runs and analyzes compilation of team management reports according to required schedules and shares data with the Team Manager
- Provides required reports (daily reports, rosters, visit notes not completed, visit reports, outcomes and other reports as requested by the team manager and office manager
- Runs unverified visit report daily to reconcile visits that have been scheduled but not completed/verified
- Verify paper visit notes on a daily basis
- Screens calls/questions from the community
- Supports effective communication between VNSW staff and patients, caregivers and other community persons.
- Utilizes agency resources to respond to customer questions
- Effectively handles phone calls and pages, problem solving where possible
- Maintains positive customer service by providing a responsive, knowledgeable and accessible approach to all customer contacts.
- Coordinates schedule with Supervisor of Support Staff to provide seamless coverage of clinical operations. Ensures continuous coverage of team operations by scheduling with counterparts and forwarding desk calls as appropriate.
- Supports Clinical Manager with Secretarial activities, including telephone communications, typing, mailing, faxing and copying, as needed.
- Performs other duties as assigned
Qualifications:
- Able to think independently and work with minimum supervision under high activity situations.
- Able to prioritize and multi-task
- Excellent interpersonal, verbal, and written skills.
- Proficient in Microsoft – Word, Outlook, Excel, PowerPoint
- Excellent telephone and customer service skills
- Minimum of two (2) years experience in the healthcare industry.
- Self starter with high level of initiative
- Associates Degree or equivalent preferred
- Ability to navigate, search information, and print reports in EMR system
- Problem solving ability
- Organizational ability
- Positive, approachable demeanor
- Understanding of business protocol
