What You'll Do

  • Assist in scheduling and tracking all consumer medical appointments-both internal and external-and ensure records are updated accordingly. Transport consumers to scheduled or urgent appointments as directed by the Care Manager, Program Director, or Nursing Supervisor. Communicate upcoming appointments to consumers, program staff, and guardians.
  • Maintain up-to-date consumer records, including PCP Team packets, treatment plan addendums, and demographic updates, ensuring accuracy in both group books and master files. Provide current demographic information to the appropriate HNBHS administrative staff.
  • Support Care Managers, Program Directors, and the Nursing Supervisor in requesting, tracking, and maintaining all required consent forms for treatment and related correspondence.
  • Coordinate and track the ordering of routine and as needed laboratory tests, assessing for abnormal results and notifying the Nursing Supervisor as appropriate.
  • Maintains consumer records - meeting all appropriate standards according to HNBHS and all regulatory bodies. Periodically audits records as directed.
  • Track prescriptions, make copies and ensure a copy is available in the designated area within the Medication Room.
  • Prepares the necessary paperwork for all medical appointments.
  • Coordinate and triage acute medical concerns.

Work Schedule

Monday-Friday 9:00am-5:00pm

Qualifications

  • Associate's degree in Human Services or a related mental health field preferred, or Certified Medical Assistant (CMA).
  • Minimum of one year of experience in a mental health or rehabilitation setting.
  • Proven ability to provide person-centered care, supporting individuals with mental health treatment needs across physical, emotional, social, and vocational domains, and adapting responses and treatment plans to meet their unique needs.
