Schedule: MondayFriday | 8:00 AM 5:00 PM

Your Responsibilities:
  • Provide personal care for patients (baths, grooming, oral hygiene, etc.)

  • Assist with dressing and undressing , as assigned

  • Ensure patient safety and use of medical equipment properly (e.g., O2, side rails)

  • Help with meal planning and preparation ; shop and feed patients as needed

  • Take and record vital signs (temp, pulse, respiration, blood pressure) as instructed

  • Monitor and report skin condition and any signs of breakdown

  • Communicate significant changes in patient condition to the Case Manager

  • Provide respite care to relieve family/caregivers when appropriate

  • Be mindful of family dynamics and interpersonal needs in the home

Qualifications:
  • At least 16 years old

  • State-certified nurse aide (STNA/CNA)

  • Current CPR certification

  • Listed in the state aide registry with a clear record

  • Comfortable providing end-of-life care and understanding of hospice philosophy
