Responsibilities: The Market Care Transition Nurse - RN is responsible for the promotion of quality, cost-effective services and care through a collaborative process that coordinates, monitors, and evaluates services provided to all transitional patients admitted to nursing centers within a specific geography.

*Reports to Market President
*Identify practice patterns within the nursing centers that contribute to high resource utilization and monitor changes in patient status that could lead to hospital readmission and report to the nursing team.
*Develop and implement effective discharge planning to improve both clinical outcomes and customer satisfaction.
*Build and sustain market hospital partners and home health service agencies.
*Utilize the appropriate National Population Health leader to grow strategic relationships and clinical programing. Qualifications: *RN license is required. *Must hold a RN license in good standing with the state board of nursing where job duties are performed. *Bachelor of Science in Nursing (BSN) is required. *Prior experience in utilization review, case management, public/population health or discharge planning is preferred. *Experience coaching clinical teams is required. *Case management experience is preferred. *Experience in business/development is preferred. *Prior experience in utilization review or discharge planning is preferred.
