Offer Based on Years of Experience

Salary range: $85,000-$95,000/yr.

Strong Hospice experience Required

Clinical License in MN (LPN, LVN or RN strongly required)

This is a field position in the St. Paul, Minnesota market

What You Need to Know:

As a RN Liaison- Care Continuum you will partner extensively with both our home health and hospice care teams to ensure seamless transitions and high-quality care across the continuum. Strong clinical experience in hospice is essential for sucess. The ideal candidate must be comfortable working in the field and engaging daily with patients and families on a daily basis.

Qualifications:

Be the Best RN Liaison- Care Continuum You Can Be

If you meet these qualifications, we want to meet you!

  • Current licensed RN/APRN/MSW in MN
  • Minimum of 5 years of direct clinical care experience in community-based setting.
  • Knowledge of CBT, motivational Interviewing, mindfulness, and solution-focused clinical interventions
  • Excellent listening, written and verbal communication skills.
  • Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks.
  • Advanced organizational skills, self-starter, proactive, creative problem-solving skills.
  • Subject matter expert on advanced illness/serious illness care, home health and hospice benefit structures, and evidence-based clinical practice.
