Job Summary and Qualifications Responsible for coordinating all aspects of the delivery, facilitation and improvement of high-quality clinical care of the structural heart patient. The Navigators primary function is to build a relationship with patients and physicians, to coordinate appointments, transportation, education, provision and/or enablement of support services and representation within the multidisciplinary environment. On an aggregate level, the Navigator will tack performance of the program in line with HCA, division and facility goals (growth, quality, practice guidelines, etc.) and identify opportunities to streamline care practice, through development of evidence-based guidelines. The Navigator will serve as a liaison between the patient and family, primary care physician, structural heart providers and staff, internal and external care providers, specialists, support network members (social workers and the wider healthcare community. This role will include conducting internal and external outreach and marketing. The Navigator conducts phone follow up within 72 hours to reinforce discharge instructions and perception of care. The Navigator will work within the facilitys clinical practice guidelines, policies and procedures, and will follow recommendations and guidelines set forth by the programs medical directors. The Navigator participates in the Performance Improvement program in the development of and compliance with treatment guidelines and all other duties assigned. These duties include but not limited to: completing KCCQ surveys, maintaining registry, submitting NCD worksheets, attend weekly valve meetings, quarterly structural heart meetings, track patient testing and maintain spreadsheet following patient through to procedure, uploading imaging to vendors, multi-campus position with time at Research Medical Center and Overland Park Regional Medical Center navigating patients.

What qualifications you will need:

  • ADN or BSN RN required
  • Advanced Cardiac Life Spt must be obtained within 6 months of employment start date
  • Basic Cardiac Life Support required upon hire
  • (RN) Registered Nurse in MO
  • A minimum of 1 year of cardiovascular nursing experience required, (2 years of experience preferred).
