PrimaryOne Health is currently seeking a Lead Patient Navigation Specialist for our Columbus health centers.

Essential Job Responsibilities:

  • Collaborates with Patient Services Manager, and Director of Patient Services to ensure scheduling procedures are meeting patient needs.

  • Provides ongoing training to Patient Navigation Staff to ensure complete and accurate data entry at registration.

  • Provides monthly agent phone reports to Patient Services Manager.

  • Updates patient information, advises patient to bring current picture I.D., updates financial information, updates sliding scale status, verifies indigent (HML) status, UDS metrics, and advises patients of balance due.

  • Acts as resource for Patient Navigation Staff when they encounter patient issues such as customer complaints.

  • Communicates patient concerns to Patient Services Manager resolution, and/or process improvements.

  • Acts as a resource and point of contact for all Patient Navigation Specialists for questions, and provides necessary assistance.

  • Pulls medical records to check on authorization for medication refills.

  • Completes medications refills according to protocol.

  • Follows PrimaryOne Health's Scheduling Telephone Triage for medical urgency scheduling.

  • Consistent attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.

  • Other duties or special projects as assigned.

Successful candidates must possess a high school diploma or equivalent. One year's experience in a medical office, customer service or reception, preferably in medical office. Bilingual capabilities a plus.
