JOB SUMMARY: Provides leadership to the health care team in the provision of psychosocial support services to patients and families. Provides Clinical Social Work consultation, counseling, and follow-up services according to psychosocial assessment and service plan.

REQUIREMENTS:

• Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline
• LMSW/LGPC
• 3-5 years of experience
