Job Description
Licensed Practical Nurse (LPN) Home Health
Location: Eugene, OR
Location Type: Hybrid In-Home Patient Visits
Work Type: Full-Time

In this field-based position, youll work independently and collaboratively to provide direct nursing care, support patient recovery, and help individuals maintain their independence and quality of life.
Compensation: Starting at $35/hour, with flexibility based on experience
Coverage Area: Eugene, OR and surrounding communities
Key Responsibilities
Deliver direct patient care in accordance with individualized care plans
Monitor vital signs, administer medications, and report changes in condition
Provide wound care, catheter care, and other hands-on procedures as needed
Educate patients and families on treatment plans, health goals, and self-care practices
Collaborate with RNs, therapists, and the interdisciplinary team to ensure quality and continuity of care
Maintain timely, accurate documentation of care provided
Qualifications
Active LPN license in the state of Oregon
Minimum of one (1) year of LPN experience (home health experience a plus)
Current CPR certification
Valid drivers license and reliable, insured transportation
