  • Revisit / DC Visit: $90$95
  • Recertification: $110
  • Discharge OASIS: $95$10

What Youll Do

  • Conduct comprehensive in-home occupational therapy evaluations
  • Develop individualized treatment plans tailored to functional needs
  • Implement therapeutic interventions to improve daily living skills (bathing, dressing, cooking, mobility, etc.)
  • Educate patients and caregivers on adaptive strategies, safety techniques, and equipment use
  • Monitor progress and update plans as needed
  • Document all care accurately and in a timely manner
  • Collaborate closely with the interdisciplinary clinical team to ensure cohesive, high-quality care

Qualifications

  • Graduate of an accredited Occupational Therapy program
  • Active Arizona OT license (or eligibility to obtain)
  • Minimum 1 year of OT experience
  • Current CPR certification
  • Valid drivers license, reliable transportation & auto insurance
  • Strong communication and documentation skills
