Salary Range: $85,000 $95,000
Schedule: Full-time | Home visits required | Occasional on-call
About the Opportunity
RCM Health Care Services is recruiting on behalf of our client, a well-respected PACE (Program of All-Inclusive Care for the Elderly) organization in Oceanport, NJ , for an experienced Registered Nurse Care Manager .
This position offers an excellent opportunity for an RN who is passionate about providing comprehensive, person-centered care for older adults in both home and clinical settings. The RN Care Manager plays a critical role in assessing participant needs, developing individualized care plans, and collaborating closely with the interdisciplinary team (IDT) to ensure quality outcomes and continuity of care.
Key Responsibilities
Provide nursing care in the center, clinic, contracted facilities, and participants homes in accordance with each participants plan of care.
Conduct thorough nursing assessments and document findings in compliance with all regulatory and organizational standards.
Participate in the 24/7 on-call rotation for participant triage and follow-up.
Coordinate home care services and monitor participants acute and chronic care needs to ensure appropriate interventions and continuity of care.
Educate participants, caregivers, and families on self-care, medication management, infection control, and healthy living practices.
Collaborate with the IDT to develop, implement, and revise individualized care plans.
Delegate and oversee tasks performed by clinic, center, and homecare aides.
Participate in quality improvement projects, utilization review meetings, and interdisciplinary case conferences.
Maintain accurate and timely medical documentation.
Qualifications
Current New Jersey Registered Nurse (RN) license in good standing.
Bachelor of Science in Nursing (BSN) preferred.
BLS certification (must be obtained within 90 days of employment).
Minimum of one year of RN experience required.
Experience with frail or elderly populations strongly preferred.
Background in home care, long-term care, or managed care preferred.
Must have reliable transportation, a valid drivers license, and state-required auto insurance.
Must be medically cleared for communicable diseases and up to date on required immunizations.
Must successfully complete a comprehensive background check and drug screening.
