Position Summary
The Assistant Chief Population Health Officer (ACPHO) is a key physician leadership role that will drive the Guthrie Clinic's transition to value-based care and improve the health outcomes of the populations we serve. This system-level position is focused on elevating performance on ambulatory quality metrics, leading data-informed care transformation, and integrating outreach and chronic disease management programs across the continuum of care.
This role will be 60% clinical / 40% administrative (up to 1 remote day/week for admin tasks).
